It may fail when the motion of nuclei and electrons becomes strongly coupled, like, in a photoexcited molecule. Two electronic states can become very close in energy and the molecule may switch from between electronic states during the nuclear motion, so the electronic and nuclear dynamics can no longer be treated independently.
