The Born-Oppenheimer approximation fails when electron-phonon coupling is strong. For instance, during high-energy collisions, fast chemical reactions, or in superconducting materials, the timescale of nuclear vibrations matches or influences the electronic state directly. In these situations, treating the nuclei as fixed or slow-moving fields is no longer physically accurate.
